At Zionsville Physical Therapy, our clinic works with most major insurance providers and offers both in-network and out-of-network physical therapy services. Coverage and benefits can vary by plan, so we encourage patients to check with their individual insurance company to understand their specific coverage and payment responsibilities prior to beginning care. We know insurance can be confusing. As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.​

​​​​Our Zionsville clinic also offers a private pay plan for those patients who do not have health insurance or who have exceeded their benefits.

 

Zionsville Physical Therapy provides first visit/ evaluation for $175, then subsequent visits for $125 per visit, self-pay fees.
