​​​What To Expect

 

At your first physical therapy visit, your therapist will take time to listen to your concerns, review your medical history, and perform a thorough evaluation of your movement, strength, flexibility, and balance. Together, you will discuss your goals and create a personalized plan of care designed to help you feel better and move more comfortably.

 

During follow-up visits, your therapist will guide you through one-on-one treatment sessions that may include therapeutic exercises, hands-on care, and education to support your recovery, while adjusting your program as you progress to ensure the best possible results.

What To Bring

  • It’s a good idea to make a list of questions that you have to discuss on your first visit.

  • List out signs and symptoms that can be of importance to the therapist. What has been hurting or bothering you? For how long?

  • Have in writing key past medical history and other important information your therapist should know or be aware of. 

  • Current medication list.

  • Please call our office at (317) 873-2033 to verify your insurance benefits beforehand.

  • Bring a copy of your insurance card and a photo ID with you.

  • Please bring the prescription for physical therapy that your doctor or healthcare provider gave you (if you have this)

  • Please arrive 15 minutes early for your first appointment to fill out required forms on the computer and in person (if you have not done prior to appointment). ​​​​​

  • In case of an automobile accident or worker’s compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number.

What To Wear

Dress comfortably in clothes that allow easy access for the therapist to examine your problem areas. 

For treatments, loose-fitting clothes that are comfortable for gentle exercises and supportive sneakers are often good choices to wear.  Bring shorts or exercise clothing to change into so that you will be comfortable and can get the most from each session.

 

Is a Referral Needed

 

You have direct access to physical therapy in Indiana, since 2014.  Most commercial insurances do not require a physician’s referral, however, some are beginning to require it. On a federal level, Medicare patients are required to have their physician participate in the plan of care, so we require that each Medicare recipient have a prescription from their physician for physical therapy.
